New Dawn Ealing, based at the Cygnet Hospital Ealing, is a 9 bedded unit for women who have been diagnosed with Borderline Personality Disorder (BPD) or with similar traits. They admit clients on a short-term crisis intervention basis as well as those for whom longer term therapy is necessary.
 
Treatment is based on Dialectical Behaviour Therapy (DBT) - a therapy devised specifically for BPD.

The treatment programme

Dialectical Behaviour Therapy is divided into skills training and individual therapy. The former takes place in group sessions and looks at:
  • Core mindfulness
  • Distress tolerance
  • Emotion regulation
  • Interpersonal effectiveness

In the latter, clients consolidate what is being learnt in skills training by focusing on positive changes in their behaviour, establishing targets and monitoring progress.
 
The ward also offers relaxation therapy, pampering groups, drama, art and music therapy.
 
Clients are empowered to make suggestions about activities through weekly community meetings.
 
Assessment and management of risk form a vital and integral part of the programme and these are regularly reviewed.
 
Clients are required to attend all therapy programmes and comply with all medication. If at any stage there are concerns about a client's attendance record, then her commitment to the programme will be re-evaluated.
 

Inclusion criteria

Individuals must be:
  • Females, aged 18-65
  • Diagnosed with borderline personality disorderor traits thereof
  • Informal or detained under the Mental Health Act
  • Able to recognise the need for help
  • Patients with an eating disorder may be considered if they have traits of BPD
 

Exclusion criteria

Individuals must have:
  • Severe mental impairment
  • Severe learning difficulties
  • Disorders of an organic nature
  • Medical conditions that require immediate treatment in medical/surgical units
  • Although many of their clients have a background of substance misuse, they cannot accept anyone who first requires drug or alcohol detoxifications. They are able to provide these services at their other hospitals.

Discharge

The clinic holds regular care planning meetings and case reviews in line with the Care Programme Approach (CPA).
 
After discharge, contact will be maintained with the client's community mental health care team to offer ongoing support and advice.
 

Clinical outcomes

Treatment addresses issues of motivation and capability with a particular focus on decreasing affective instability and impulsive behaviours. Research has demonstrated that DBT impacts positively and lastingly on impulsive and self-harm behaviour, as well as components of suicide in women with BPD, when compared to standard treatment.
 

Carers and leavers group

A monthly Carers Group addresses the needs of carers, allowing them to meet with one another, share experiences and better understand the needs of clients. A Leavers Group allows clients who have been discharged to visit the unit and speak about their experiences after leaving.
