Brachytherapy, a new technique in the treatment of prostate cancer
Brachytherapy means "near-therapy" (from the Latin words). It consists of implanting low energy isotopes (iodine-125), encapsulated into a "seed", inside the prostate gland, by means of needles which are guided through the perineum, using a sophisticated transrectal ultrasound system. The radiation dosage is confined to the prostate and does not affect the adjacent areas of the body, nor there is a significant radiation risk to surrounding people.
The most important advantages of brachytherapy are:
- Avoids the risks of an operation such as radical prostatectomy.
- Is an outpatient procedure; it requires just a few hours hospitalisation.
- Allows a rapid recovery and a quick return to normal activity.
- Placement of the Iodine seeds can be done in a uniform way throughout the prostate, including the apex.
- The risk of bleeding during implanting is minimal.
- The procedure is carried out just once.
- It is very well-tolerated by older patients with general poor health, as it avoids a long stay in bed.
- Sexual drive may be maintained in more than 90% of cases.
- No urinary incontinence.
- The effectiveness of the treatment is backed up by more than ten years of follow-up study in the USA (Blasco, Radge, Stone...).
Types of brachytherapy treatment
Permanent seed implants
The permanent seed implants are implanted in an hour, on an outpatient basis. Tiny radioactive sources are placed with precision inside the prostate gland, using various needles and ultrasound control. The low energy radioactivity emitted by the seeds has a very short radius distance activity, limited to the prostate. This allows very high dosages to be administered to the prostate while the level of radiation reaching the adjacent tissues, bladder and rectum, is very low.
The seeds continuously emit their radiation for weeks or months. The seeds are indicated in cases when the tumour is confined on the prostate, either in one or both lobes. Prostate brachytherapy is extremely well tolerated. Just a few hours after carrying out the implantation, the patient can be released from the clinic. The patient can return to normal life as early as next day.
High dosage rate implants
High dosage rate brachytherapy is a procedure by which the radioactive sources are temporarily placed inside the prostate. Radiation is focused directly on the cancerous zone, protecting the neighbouring healthy tissue. Radioactive sources are introduced into the prostate by means of plastic needles, placed under precise ultrasound control. By means of a complex computer program, our experienced physicists team calculate the time each radioactive source must remain in place in order to give the tumour the dosage the doctor considers necessary.
The use of complex algorithms enable the optimisation of the dosage distribution in such a way that the volume to be treated receives high radiation dosages while the quantity irradiated on healthy tissue is minimised. The treatment lasts just a few minutes, and the tubes are withdrawn. High dosage rate brachytherapy is a highly precise treatment. That is why it is the ideal treatment for complex cases, which have had previous surgery or because of their characteristics are not suitable for treatment by permanent implants.
